Ongoing follow-up visits focused on monitoring symptoms, fine-tuning medication, reviewing your response, and optimizing outcomes — with real continuity of care.

Out-of-Network Support
Superbills provided
For self-pay and out-of-network patients, detailed superbills are provided after each visit to support potential reimbursement through your out-of-network benefits.
Insurance & Reimbursement
Momentum is a self-pay, out-of-network practice and does not bill insurance directly. After each visit, we can provide a detailed superbill you can submit to your insurance company for potential reimbursement through your out-of-network benefits.
Appointment Policy
To protect access for all patients, we ask for advance notice to reschedule; missed or late-cancelled appointments may be subject to a fee. Most patients are seen monthly, with a minimum of one visit every three months.
